We've always just bought those cheap multi-colored toothpicks and let people know what color represents what. Red = rare; orange = med-rare... etc. I think they go for about $0.99 at your local grocery.
There's an episode of King of the Hill where Bobby asks Hanks what to do if someone doesn't want their steak rare. Hank says you politely ask them to leave. I have slightly similar feelings.
