Mint ice cream, vodka, blender.
The "Top Secret" guy who reverse engineers recipes uses vanilla ice cream, mint extract, milk and green food coloring. I imagine I'd add some nice irish whiskey.

Creme de menthe for color and extra mintiness.
My Mom used to make these! I'm pretty sure it was vanilla ice cream and creme de menthe in a blender, then topped with a little chocolate sauce. Us kids got the versions without the liquor, but with green food coloring (the horror!), of course.
A few years ago, I made mint ice cream (regular egg-based ice cream base, steep mint leaves for 20 minutes while the mixture is still warm, freeze in ice cream maker) then added a ton of green food coloring when making the shakes.
